Medication Form |
Administration of Medication:
Students who need to take medication MUST BRING that
medication, clearly marked with the following information:
1. Student's full name
2. The name of the medication
3. Instructions as to the amount to be taken, when, and for
how long (for example - one week)
A signed medical release
statement must be on file in the school office before any
medication can be administered. This includes over-the-counter
and prescription medications, which must come to school in the
original container.
If your child is prone to headaches, please send in aspirin,
Tylenol, etc., clearly marked with his/her name and medication
and instructions for administering it. We CANNOT provide aspirin
according to state law. Medication may only be administered in
the school office by authorized personnel or approved parent
volunteers. |
